Personalized Care
Concierge medicine—individualized healthcare tailored to a person’s unique needs—shouldn't be seen as a new way of practicing medicine. In fact, at CCMD, we believe that concierge medicine allows us to practice medicine in a traditional way that also embraces the benefits of technology like virtual visits and texting that increase access and convenience for our patients. Carlyle Concierge is also your personal health advocate, meaning we help you get the care you need, whatever the situation. Traditional practices often have hectic schedules that make it difficult to get the care you need and frustrate both physician and patient. At CCMD, our goal is to deepen that patient-physician relationship with trustworthy care from start to finish.
Meet
Dr. Amy Blossom
Dr. Amy Paxton Blossom was raised in Tupelo, MS and graduated from Tupelo High School in 1993. After earning a Bachelor of Science in Biology from Emory University in 1996, Dr. Blossom studied at the University of Mississippi Medical Center where she completed both her Master of Science in Microbiology and her Doctor of Medicine. In 2006, She was awarded the Carl Evers Resident of the Year Award while completing her residency in Obstetrics and Gynecology at UMMC.

Dr. Blossom worked as a staff physician at UMMC from 2008 to 2010, and in 2010 earned her board certification in Obstetrics and Gynecology. From 2011 to 2020, she was a physician at New Albany OB-GYN Clinic and on staff at Baptist Memorial Hospital. In 2021, she began Carlyle Concierge Medicine.
Membership
At CCMD routine care and annual health screening physician visits are covered by your yearly membership fee. Lab work performed will be billed to your insurance or may be paid outside of insurance and you are responsible for what is not covered by insurance. If you need additional testing or services, these will be coordinated through your health insurance as with other medical care you may receive.
When necessary, we offer added testing and treatments for specific health concerns. If the need for a specialist or specialized testing arises, our extensive referral network of top-tier physicians and facilities and our trusted reputation will be utilized to secure you the exceptional care you deserve.

Initial In-Person Consultation - $250
Membership - $1800 (includes Initial Consultation)

Initial consultation that includes thorough medical history and lifestyle assessment includes personalized healthcare plan covering nutrition, exercise, supplements, behavioral health, hormone therapy, and more.

Quarterly visits with physician (if needed) - may be in person, telemedicine or telephone

Extended office visits regardless of medical necessity

Is Concierge Medicine Right For Me?
Patient-Physician Communication: Under the subscription-based model of concierge medicine, you are able to communicate with your physician on your patient needs. This helps your doctor identify the risks and other factors that are unique to your body.

Accessibility: Many patients who switch to concierge medicine do so for convenience. It removes the hassle of having to wait for an appointment when sick or other needs arise unexpectedly. At CCMD, you will be seen for appointments, either in-patient or virtual, as soon as the same day.

Range of Care: At Carlyle Concierge, if we can’t provide the service you need, we will put you in touch with the physician who can. Whatever your health concern, we are here to help you, and we will try our best to provide an excellent patient experience.
